Hemorrhoids are among the most common diseases in developed industrial countries. Their prevalence reaches 120–140 cases per 1,000 adults. The average age of patients suffering from hemorrhoids is between 30 and 50 years.
There are acute and chronic hemorrhoids, which are stages of the same disease. Hemorrhoids are also classified as internal, external, or combined.
Acute hemorrhoids or anorectal thrombosis:
This condition manifests as thrombosis of the external hemorrhoidal nodes or prolapse of thrombosed internal nodes from the anal canal.
Thrombosis of the hemorrhoidal nodes is characterized by a significant increase in their size, hardening, and causing quite intense pain.
Chronic hemorrhoids: four stages are distinguished.
- First stage – characterized by periodic discharge of bright red blood and itching.
- Second stage – prolapse of hemorrhoidal nodes with spontaneous repositioning. May be accompanied by bleeding or occur without it.
- Third stage – prolapsed hemorrhoidal nodes do not reposition on their own. Bleeding may or may not be present.
- Fourth stage – characterized by the inability to reposition prolapsed hemorrhoidal nodes from the anal canal.
The treatment strategy for hemorrhoids is chosen depending on the stage. In the first and second stages, conservative therapy can be sufficient, and treatment using electrocoagulation, photocoagulation, cryodestruction, or sclerotherapy may be suggested. The application of ligatures (suture ligation or ligation with latex rings) is effective in the second and third stages, while in the third and fourth stages, the Longo hemorrhoidectomy technique may be used.
Laser Hemorrhoidoplasty is the most modern and universal method of minimally invasive surgical treatment for stage II, III, and IV hemorrhoids in patients of any age. This method allows for the removal of both internal and external hemorrhoidal nodes in the most gentle way possible. The main advantages include: reducing the operation time to 15–20 minutes, minimal complications, quick recovery, and mild postoperative pain.
Procedure
The operation is performed under local anesthesia. After suturing the vascular pedicle of the hemorrhoidal node, the node is punctured with a laser fiber, and its internal vaporization is performed. During the procedure, the mucous membrane of the anal canal remains intact. With laser vaporization, the edges of the mucosa and submucosa are sealed together, so suturing is not required, which significantly reduces postoperative pain.
Postoperative period
The recovery period is usually painless, although some discomfort in the anal canal area may be felt.
